Live Well Lincoln seeking input on Community Health Survey

Live Well Lincoln is eager to learn more about the health issues that affect Lincoln County residents. Live Well Lincoln is a local partnership with the common goal of having Lincoln County be a healthy place to live, learn, work and play. Live Well Lincoln includes partners from various sectors as well as community members. Lincoln County Health Department, Aspirus Health, UW-Madison Division of Extension Lincoln County, Aging & Disability Resource Center of Central Wisconsin and Lincoln County Department of Social Services provide oversight to the committee.
Starting this month, a community health opinion survey will be distributed throughout Lincoln County to gather information on major health concerns for community/county residents. This information will be used to help identify health priorities and help guide countywide prevention efforts for the next five years.
This survey is part of Live Well Lincoln’s Community Health Assessment (CHA) process which occurs every five years. The Community Health Assessment includes local data, community survey results and information gathered from community members through focus groups and interviews.
This information is used to develop a Community Health Improvement Plan by addressing health priorities that we can collectively work on as a county. “Community feedback has always been an important piece of this process,” says Shelley Hersil, Health Officer/Director for the Lincoln County Health Department. “In order to develop a Community Health Improvement Plan that is specific to our residents, we need to hear from you about the health challenges facing our county.”
Surveys are available at Lincoln County Health Department, local organizations, area businesses and a variety of other locations throughout the county. You can also complete the survey online at www.lincolncountyhealthdepartment.com or on the Lincoln County Health Department’s Facebook page.
It should take about 5-10 minutes to complete.
The survey is also available in Spanish. Those who fill out the survey will have a chance to win a $25 Chamber Gift Certificate which can be used at local businesses. The survey is confidential and your answers will not be shared.
